I will be participating in an individually assigned exercise rehabilitation program with an individualized home exercise program. The amount of exercise is determined by my individual abilities, based on my exercise tolerance test (exercise stress test – if applicable) and from other diagnostic test results provided by my physician (family doctor, internist). My exercise will be adjusted periodically by the Cardiorespiratory Rehabilitation Program Physiotherapy staff. My progress will be re-evaluated regularly with a physiotherapist. My health with be monitored every 4 weeks by the Registered Nurse.
During virtual rehab exercises and physical activity intensity will be monitored by the physiotherapists. You will be educated on self-monitoring. The possibility exists that physiological changes will occur during exercise. These may include: abnormal blood pressure responses, fainting, changes in heartbeat, and very rarely heart attack or death.
The medical and exercise information obtained during my participation in the cardiorespiratory rehabilitation program is strictly confidential. Progress reports will be forwarded to my physician(s); otherwise, information will not be released without my verbal consent. Data may be used for statistical or scientific purposes, but names will not be attached.

The Cardiorespiratory Rehab Team is pleased to be able to offer our services to individuals in our community with known cardiovascular risk factors and respiratory conditions. As you may know, the foundations of Cardiorespiratory Rehabilitation are rooted in behavioural modification theory. That is to say, in order for you to lower your risk of heart, blood vessel disease, and respiratory-related issues, while optimizing your health, you need to minimize unhealthy behaviours while adopting healthy ones.
Behaviour change is very difficult and requires an understanding of the benefits, risks, and diligence in addressing recommended changes over time. As a team we are dedicated to assessing your risk, providing you with an individual, evidence-informed functional, physical, and self-management profile. We will also provide you with recommendations for change. We will do our best to help you to make those changes safely and effectively.
When you agree to being admitted to the Cardiorespiratory Rehabilitation Program and working with our team, it is important that you understand the level of commitment required of you personally. Admissions are conditional on the client attending all scheduled individual virtual sessions, and group education sessions. In addition, we expect you to follow-through with our recommendations to the best of your ability and with our help. Please report how you are doing back to the team.
We understand that behavior change is a slow and difficult process, and that there may be set-backs along the way, but we do expect your greatest effort while you are here. For the majority of individuals this will involve daily walking, a 60 to 90 minute exercise program 3 or more times per week (or as prescribed), nutritional changes, attending education sessions weekly and individual counseling sessions when they are scheduled.
