Freedom of Information (FOI)

What is FOI?

As of January 1, 2012 hospitals in Ontario will be covered for the first time, by the Freedom of Information and Protection of Privacy Act (FIPPA). After that date you have the right to make a request for access to a range of recorded information held by the hospitals, including records of personal information that came into the custody or under the control of a hospital on or after January 1, 2007. Access to one’s own records of personal health information, however, will continue to be governed by the Personal Health Information Protection Act (PHIPA).

For more information on rights to your personal information read our privacy policy.

How do I make an FOI Request?

In many cases, it may not be necessary to make a formal request to get access to hospital records. The LACGH is committed to proactively sharing information with the public through this website.

When information is not available on this website, a formal written request for information can be made to the Freedom of Information Coordinator at Lennox & Addington County General Hospital.

Under FIPPA, an access request must be made in writing and provide sufficient details to enable the hospital to identify the record(s) requested. Please ensure your contact information is included. All requests must be accompanied by a $5.00 application fee (a cheque or money order payable to “Lennox & Addington County General Hospital “) Do not send cash. If additional costs are incurred by the hospital related to processing the request, it is the responsibility of the requester to cover these costs. (i.e. photocopying) You will be notified in advance of these costs before proceeding.

You can request access to general records or personal information (information about yourself, other than health information). For a list of types of personal records see the LACGH Personal Information Bank.
